Eligibility information
Clients are only accepted through referrals from HRA’s PACT System with a 2010e housing application form. If you have visited an ER more than five (5) times or have been hospitalized two (2) of more times within the last 12 months, you may qualify. Clients in this program are placed in housing that can be in any of the five NYC boroughs.
(1) Single Adult ONLY
Not eligible for other supportive housing options
Homeless or at risk of unstable housing
Diagnosed with two or more chronic conditions
Diagnosed with one condition and be high-risk for another
Diagnosed with one serious condition in addition to a persistent mental health condition
