OASAS MRT Scatter-Site Housing Program
Similarly to Health Homes MRT, this OASAS program provides scattered-site apartments to high users of Medicaid who are homeless, however this program is specifically for those living with a qualifying substance use disorder. Clients are only accepted through referrals from HRA’s PACT System with a 2010e housing application form.

Eligibility information
While you do not need to be diagnosed with a chronic condition or mental health condition for this program, if you have visited an ER more than five (5) times or have been hospitalized two (2) of more times within the last 12 months, you may qualify for OASAS MRT Housing. Clients in this program are placed in housing ONLY within the Brooklyn borough.
